Denise Richards<\/a> previously dated Patrick – shared the accompanying handout on her Instagram along with the caption: “Miss u so much Patrick”.<\/p>\nThe front of the flyer bore the words, “Celebration of the spirit of Patrick Muldoon” over a photograph of the actor wearing a suit, with the church details and date underneath.<\/p>\n
\nThe Days of Our Lives star’s death certificate was released at the end of April and revealed his remains had been cremated on 28 April.<\/p>\n
\nThe document also confirmed he had passed away due to myocardial infarction – a heart attack – with pulmonary embolism and hereditary coagulopathy listed as underlying causes.<\/p>\n
\nA pulmonary embolism is “when a blood clot blocks a blood vessel in your lungs”, according to the National Health Service (NHS).<\/p>\n
\nCoagulopathy, meanwhile, “happens when there\u2019s a problem with how your blood forms clots”, Cleveland Clinic states.<\/p>\n
\nSami, whose father is actor Charlie Sheen, had previously shared her sadness over Patrick’s death.<\/p>\n
\nSami took to her Instagram Story to share photos and video clips of Denise and Patrick and wrote: “Patrick was someone that everyone loved. he was so quick witted and humble but still made sure everyone knew he was a rockstar and never cared what people thought about him. from day 1 till the last day I saw him, he’s never changed.<\/p>\n
\n “I’ve been lucky enough to grow up with him and i have yet to live my life without him. the friendship him and my mom had was so beautiful + something not many people get to experience.(sic)”<\/p>\n
\nSami insisted Patrick will be “missed every single day”.<\/p>\n
\nShe concluded: “I hope i wake up from this nightmare soon.”<\/p>\n
\nSami then shared another Instagram Story featuring what she felt was a message from Patrick.<\/p>\n
\nAlongside a photo of a heart-shaped cloud in the sky, she wrote: “Right after he passed i was sitting outside begging for a sign from him to let us know that he\u2019s okay and made it to the other side, and then a couple minutes later i looked up and saw this in the sky. can’t be a coincidence.(sic)”<\/p>\n
\nDenise and Patrick first met in an acting class in the 1990s before starring together in the 1997 film Starship Troopers. <\/p>\n
\nThe pair dated for nearly five years before remaining close friends and collaborators, most recently working together on the upcoming crime thriller Dirty Hands.<\/p>\n
